If you participate with an HMO and are referred to a specialist, all you will need to do is call our managed care department 24/7 with your appointment date.

You may leave your message on the referral voice mail including your name with spelling, date of birth, name of the specialist you will be seeing, the location of the office, the appointment date, the type of insurance you have, and a daytime phone number. Please allow at least 5 days to complete your referral.
